Welcome to the 2014-2015 Student Insurance Plan Information Page
All Registered students taking 12 or more credit hours, including off campus students taking Internet courses, are automatically enrolled into Plan B at registration. All other undergraduate students taking 6 or more credit hours and all graduate students taking credit hours are eligible to enroll into Plan B on a voluntary basis.
Students must actively attend classes for at least the first 31 days
after the date for which coverage is purchased. Home study and
correspondence courses do not fulfill the Eligibility requirements that
the student actively attend classes. The Company maintains its right to
investigate Eligibility and student status and attendance records to
verify that the Eligibility requirements of this Policy has been met.
If the Company discovers the Eligibility requirements have not been
met, its only obligation is to refund premium.
Eligible students who do enroll may also insure their Dependents.
Eligible Dependents are the spouse and unmarried children under 19 years
of age who are not self-supporting. Dependent Eligibility expires
concurrently with that of the Insured student.
Waiver / Enrollment Process
All Registered students taking 12 or more credit hours covered by other comparable health insurance, may decline the Coppin State University student health insurance by completing the Online Waiver form. For your waiver request to be considered, you must complete this form by the waiver deadline of October 1, 2014. Click here for instructions on HOW TO COMPLETE THE WAIVER FORM. If you are experiencing a problem with the waiver site, call (440) 827-2841. Once the waiver request is reviewed, students will be notified via email whether their waiver request has been approved or denied. Waivers will not be accepted after the deadline. The premium cost for this plan will remain, non-refundable on your tuition bill. Waivers received in the fall apply for the full year. Only new incoming students for spring will need to waive coverage for Spring/Summer.
All other students students taking 6 or more credit hours and graduate students and dependents of enrolled students may elect to purchase this plan on a voluntary basis by clicking on the Online Enrollment page.
CareFirst® BlueCross® BlueShield®
SEBT has arranged for you to access the CareFirst® BlueCross® BlueShield® Preferred Network in your local campus community. Using our Participating Provider Network you may access in-network savings for services rendered nationally if you require treatment or hospitalization outside the immediate area of the Coppin State University campus. NOTE: From there you will select PPO Plans, then select BlueCross BlueShield Preferred and then select Regional Network - MD, DC & No VA.
ID cards will be issued to students after the waiver/enrollment deadline.

Plan A is a prepaid Coppin State University Community Health Center plan. Provides 100% for all services when rendered within the community health center.
Plan B Silver Plan provides a $50,000 annual maximum on covered medical services, 100% for services rendered within the Coppin State Community Health Center and access to the CareFirst BCBS provider network.
Plan C Platinum Plan, inclusive of Plan A, offers an unlimited lifetime and annual coverage for covered medical services,
100% for services rendered within the Coppin State Community Health Center and access to the CareFirst BCBS provider network.
All plans includes a $10,000 Life and $10,000 Accidental Death and Dismemberment Benefits.
